While we used to open hands like North, we have become slightly more conservative in recent times (mostly to avoid the requirement that we alert/pre-alert all our 1M openings). So:
IMPrecision:
1♣ (16+) - 1♥ (spades)
1♠ (rel) - 1nt (diams or balanced or 3-suited)
2♣ (rel) - 2nt (short hearts)
3♣ (rel) - 3♦ (5143)
3♥ (rel) - 4♣ (5 RP)
4♦ (rel) - 4♥ (0/2 top spades)
4♠ (rel) - 5♣ (1 top diamond, no top club)
6♠
This sequence resolves basically responder's whole hand. Note that once we know 5 RP, it is difficult to construct a hand where slam is worse than on the heart break, so we might as well continue relaying.

If we passed,

1♥-1♠ 4+ spades, usually GF; ask
2♠-2N secondary diamonds and heart shortness, GF; ask
3♦-3♠ 5143; RKC♠
3N-4♣ 1/4; Q?
4N-6♠ yes and K♦; ok
If we needed a swing, we could bid 6N on the latter auction (protected K♦), but spades can handle a bad heart break and with the strong hand looking at all the good spade spots it seems safest.

Why does TOSR go from skipping to show to skipping to deny? 4 or more vs 3 or less holdings?

The annotation was on the wrong line. 4♠ showed ♠, ♦ and denied ♣. Now, 5♥ denies the singleton ♥ honour and shows second ♠.
