Not sure where the wheels came off for Z-T. In a prior hand they used a puppet that would then allow opener to RKC in a specific suit. So 3S was self-alerted as a spade ask which doesn't make a lot of sense if North's pattern was really 1534. Assuming 3N accepted the puppet and 4S asked for spades, North's bidding makes sense.

I only count 5 QPs in North. Also, what would 3 show, 5 QPs?

Corrected -- that's right, 3 would show 5 QPs and 3N would be 6-7. Basically, responder doesn't zoom past 3N with < base+3. If there's a subsequent ask, the 6 QP hand bids the first step and the 7 QP hand zooms into DCB.

Two-way pass:

pass 1
1NT 2
2 3
4

pass = 0-8 or 15+
1 = nat 5+ cards, 9-14
1NT = strong variant, balanced hand
2 = natural, min/max still undisclosed
2 = natural, forcing
3 = min hand, not interested in slam...
4 = ok lets stop here

As for what went wrong in Z-T, I think opener's hand devaluates significantly after hearing of the spade singleton. Plus, he's got a couple of worthless jacks. I would just sign-off in 4 after 3. If responder can't find a bid after this, it's because slam is a mirage. Hope this helped.
