Well,
when a player bids 2 after his partner's 1NT opening bid and this is not alerted you will possibly suspect an irregularity.
When he turns up as Dummy holding less than 2 Diamonds and 5+ hearts you are pretty close to having a confirmed irregularity.
And when the opener admits he should have alerted the 2 bid there is no longer any doubt.

If a player fails to follow suit during the play you may suspect an irregularity, but when it subsequently turns out that he had a card in that suit the irregularity is certain.
