From Law 49:
or when a defender names a card as being in his hand, each such card becomes a penalty card (Law 50)

When a defender designates one card and then is allowed to change his designation. Do you apply Law 49 on the withdrawn designation and in case how?

No, of course not. He has not named a card as being in his hand so Law 49 is irrelevant.

You are allowed to change a designation is it was unintended. So, if you bid 3 but intended 3 you are allowed [subject to other considerations] to change it to 3. You have not bid 3, you never intended 3, and the unintended 3 'bid' has no informational content.

Similarly if you designate the 3 to be played but intended the 3 you are allowed [subject to other considerations] to change it to the 3. You have not played the 3, you never intended the 3, and the unintended 3 'designation' has no informational content, and may or may not be in the hand.
