Assuming 1NT was 15-17, what is West doing inviting 6NT on a balanced 14 count? (and not a very good one either)

Well, he does have AKQxx of a suit, so there is some merit in accepting, particularly if your style is "invite conservatively, accept aggressively". There's no merit at all in the 4NT bid.

No blame.

West thought they were playing 17-19 NT, and East thought they were playing 13-15. I am sure that once both partners agree on the strength of their 1NT opening there will be no more problems.
That's impossible. No one can give more than one hundred percent. By definition that is the most anyone can give.

I will assign the blame to West. He has a simple 3NT bid.

In my opinion, East's acceptance isn't that bad with that club suit. On the bidding West didn't try to find a major suit. It looks like he has diamond length. Two minor suits running would make 6NT easy.

I think Noble nailed it. East's bid was questionable but had merit. West's bid had no merit.

West 100%.

East 75%.

West had no business bidding 4NT.

East should have passed 4NT; however, if he is going to accept, he should bid 5 to suggest that a slam in clubs might be a better option than 6NT.

I voted "other opinion" since I find fault with both partners.
