Don't see how a passed hand can make a penalty double of 1NT just because partner has made a 1 level overcall. Depending on your agreements about takeout doubles, perhaps the doubler has a 4234 10 count. Or 4144 10 count? Or perhaps the doubler passed a 13 count, and trusts you to have 10HCP for a 1♥ overcall. Who knows, but for me the double is impossible.

This was a bit of a hypothetical. there was no double at the table (there probably could have been a 1♠ advance), I was just curious if anyone had given any thought to what it would mean if it were made or what they would do if p were crazy enough to drop one on us... . The full deal was:

Partner has a good hand with a heart void. Wouldn't you pass over the negative double with that?
Of course he has no idea if he can come close to beating 1NT so DBL cannot really be "penalty". But one thing he knows is that, if he passes now, the likely heart opening lead against 1NT will not be a good start for the defense.
So he is not making a penalty DBL - he is just describing his hand. The overcaller is welcome to do whatever he wants (and if he sits he has been warned against leading a heart).

As for the actual hand, I'd have bid 1♠ over the negative double.
Fred's suggestion makes some weird sense maybe, but I'm pretty sure I'd find a bid with such a hand on the previous round. KISS
